Required Information:

Your name, address, telephone number, and an emergency contact person.

The exact location and entrance we will pick you up at.

The date and time for the requested pickup.

The address, phone number and contact person at your destination.

Any special accommodations needed.

We will make every effort to meet your specific needs and requests.

Cancellations

Our service to you is very personalized and pre-scheduled with our para transit vans. Please notify us promptly of any cancellation.

We offer a 'No-Charge' cancellation policy if a pickup is cancelled anytime prior to 30 minutes before the scheduled pickup. If cancellation is not received prior to that time, the client is billed for the scheduled trip.

Personal Attendant

A personal attendant may accompany the client at no charge. Please identify the attendant at time of scheduling.

Companion

One family member or companion may accompany the client at no charge. The companion must have the same pickup origin and drop off destination as the client.
